Treatment & Aftercare Program

At Canadian Health Recovery Centre, we support our clients in building an aftercare plan and setting obtainable goals for lasting recovery.  Our treatment team offers clients a monthly check-in to evaluate their aftercare plan, and support clients in making modifications to their aftercare plan when needed. Continuity of care is essential to our inpatient residential treatment program.

Throughout treatment our clients will be meeting with our interdisciplinary team and together, with the client will develop an aftercare treatment plan prior to their discharge date.  We realize that many of our clients come from all over Canada and other parts of the world to attend our program therefore we have incorporated telephone and/or Skype counselling to provide flexibility and unbiased access to aftercare.

CHRC aftercare treatment plan includes:

  • Telephone and/or Skype counselling
  • Referrals and resources for relapse prevention and continued care
  • Referrals for housing and debt management concerns
  • Nutritional support and follow-up information in client’s hometowns
  • Nutritional discharge planning, including meal planning and a CHRC Recipe Book
  • Fitness support and information for local gyms and personal training in client’s hometowns
  • Family support and counselling opportunities

  • Family support and counselling opportunities
  • Community counselling resources as well as private counselling resources
  • Weekly alumni support group
  • Monthly Alumni follow up calls to check in on the execution a clients aftercare plan for the first year post- treatment. *clients will be contacted weekly for the first month, and monthly for the next 11 months
  • An open invitation for clients/families to speak with available staff by phone for support post treatment.

Telephone-based continuing care does not require transportation or interfere much with work or childcare responsibilities, and this flexibility may help patients stay engaged in recovery and maintain the gains achieved during inpatient treatment.
